Sharesource Connectivity Platform: connecting patients, clinicians and PD technology for proactive treatment decision-making

Strengthened by Sharesource Connectivity Platform technology, our automated peritoneal dialysis (APD) therapy is designed to be user-friendly and data-rich. The Sharesource Connectivity Platform gives clinicians on-demand access to accurate and timely treatment data, notification of potential issues and remote adjustment of device settings and treatment prescriptions, as well as enabling clinicians to model PD prescriptions based on individual needs with the Sharesource Adequest modeling tool. Importantly, clinicians have greater visibility to a patient’s adherence patterns, which may allow for earlier intervention and give patients peace of mind, knowing that their care team is monitoring their daily treatments.

The ICU is a complicated clinical care environment, and efficient data management is essential for optimal treatment delivery. Our TrueVue digital solutions help bring greater efficiency and proactive management to CRRT programs. TrueVue technologies were specifically developed to support effective therapy delivery in the ICU by enabling wireless EMR connectivity and the creation of patient profiles, which streamlines therapy administration. Through TrueVue Therapy Management, we work with hospitals to analyze CRRT performance to develop quality improvement programs to deliver on efficiency goals.

Continued digital advancements

We are continuing to build on our proven digital connectivity innovations to help enable efficiencies for dialysis providers and make dialysis therapy easier to manage for both patients and clinicians. Planned enhancements to Sharesource Connectivity Platform, our home patient management solution for PD, are aimed at increasing proactive patient and clinician engagement, improving clinician workflows and empowering patients to stay on their chosen therapy. In addition to EMR connectivity and Black Box Data Mining (BBDM) Analytics available today with our hemodialysis (HD) machines, we are developing a suite of digital capabilities to support improved HD clinical workflow efficiency. And in the acute setting, we are exploring partnerships to develop meaningful tools for optimizing workflow in the ICU.
